3. Appointment Requests

An online request is not a confirmed appointment. An appointment is confirmed only after our staff communicates confirmation. Availability, clinicians, services, and hours may change. We may request additional information before scheduling or providing services.

4. Eligibility and Accurate Information

You must be at least 18 years old to use the website independently. A parent or legal guardian may use it on behalf of a minor. You agree to provide accurate, current, and complete information and not to impersonate another person or submit information without authorization.

8. SMS Messaging Terms

Program Description

If you provide your mobile number and affirmatively opt in, Hispana 360 CARE may send SMS messages related to appointment scheduling and confirmations, appointment reminders, follow-up requests, clinic information, service updates, account or billing notifications, and other healthcare-administration communications. Promotional or marketing messages will be sent only when you have provided any consent required by law.

Consent

By opting in, you authorize Hispana 360 CARE and service providers acting on our behalf to send SMS messages to the mobile number you provide. Consent to receive SMS messages is not a condition of purchasing or receiving healthcare services. You represent that you are the subscriber or customary user of the mobile number provided and are authorized to consent.

Message Frequency and Charges

Message frequency may vary. Message and data rates may apply. Your mobile carrier's terms and charges govern your wireless service.

Opting Out

You may opt out at any time by replying STOP to an SMS message. You may also communicate a clear request to stop messages by another reasonable method, including calling (936) 242-6987 or emailing management@hispana360.com. We may send one non-promotional confirmation message after an opt-out request. After processing the request, we will stop messages that require consent unless you later provide new consent. Opting out of SMS does not prevent us from communicating through another lawful method when necessary.
